EDUCATIONAL NEED
Pulmonary arterial hypertension (PAH) is a life-limiting condition that significantly impacts quality of life. Attention to PAH from the medical community has grown substantially over the past two decades, driven by an increased understanding of the genetic and pathological drivers of the disease and the subsequent availability of new treatment options. With the expansion of PAH medications, long-term patient outcomes and quality of life have dramatically improved. Clinicians responsible for the management of patients with PAH must have a thorough understanding of PAH medications. Pharmacists in particular must be adept at transitioning care between the hospital and community settings, which requires education and collaboration with all healthcare providers involved in the care of PAH patients. This online modular activity provides clear and concise discussions on several relevant topics for pharmacists regarding the ongoing management and transitioning of care for their patients with PAH.

TARGET AUDIENCE
This activity meets the needs of pharmacists in a variety of practice settings, including large and small healthcare systems, outpatient clinics, managed care organizations, long-term care facilities, and academia. This program targets pharmacists who are responsible for the safe and effective use of medications utilized for the treatment of patients with PAH.

 

 

LEARNING OBJECTIVES
Pharmacists must be adept in ensuring patients with PAH receive optimal care as they transition between hospital and community settings. Upon completing this program, participants will be able to:

  • Recognize patient risk stratification tools to guide PAH therapeutic approaches
  • Differentiate among the pulmonary arterial hypertension (PAH) medications based on pharmacology, therapeutic target, dosing, safety, and efficacy
  • Recognize the role of pharmacists in transitioning PAH patients between home/community and acute care settings
